Free School Sports Physical

Jersey Shore Area School District offers free sports physicals on Wednesdays in June.

To schedule a physical, you will need to complete the following:

Complete the Sports online registration (found by clicking the button below) and pay the sports fee.
Print and complete all three pages from the Free School Physical Forms link below and return to the nurse’s office.
Schedule an appointment when you return the papers to the school nurse or call 570-398-7400 ext. 1010.
