Volunteer Forms

The Jersey Shore School District values the contributions made by the volunteers in our schools. 
State law requires that volunteers must obtain background clearances.  This impacts many of our volunteers.  To better understand the laws and policies that govern volunteers, click on the links provided below.  School Board Policy may be more restrictive than the law, but cannot be more lenient than the law.

Volunteers in the Jersey Shore Area School District must submit the following before they will be permitted to volunteer within the district.

  1. JSASD Volunteer Application
  2. Act 34 State Policy Criminal History Record
  3. Act 151 Child Abuse History Clearance
  4. Tuberculosis Test
  5. PDE-6004 Form
  6. Federal Criminal History Record (For individuals who HAVE NOT resided in Pennsylvania for the past 10 consecutive years.)

Take completed application, original clearances, tubersulosis test results and PDE-6004 form to the school office. All completed paperwork must be submitted together at one time.

Volunteer clearances must be renewed every five (5) years. The Volunteer Affirmation must be submitted in the years that clearances are not required to be renewed.

The Federal Criminal History Background Check is required only for individuals who HAVE NOT lived in the state of Pennsylvania for the past 10 consecutive years.
