High School Transcript Requests

By completing and signing the Transcript Request Form (links below), the student of the application is authorizing Jersey Shore Area Senior High School to send that student’s transcript to any post-secondary institution(s) or prospective employer(s). Please be sure to submit your name as it was when you attended high school (ie. maiden name). 

Transcripts contain a record of high school courses and grades, class rank, GPA, attendance records, and may contain SAT/ACT scores if particularly requested on the form.   Student Records are considered confidential information and are not released to outside agencies (employers, college admissions offices, etc.) without a student’s or parents’ written permission. Please note: after a student has turned 18 records are only released with the student’s permission.

A response to your request may take up to 3 business days.
