ALL PHYSICALS for the 2023-2024 Sports Seasons MUST be dated after June 1, 2023.

Complete the online registration following the directions in the the link below.

Registration, physicals, and payment for a Fall sport are due August 1, 2023.

Physical Forms (link below):
1. Complete Section 5.

2. Take both Sections 5 & 6 to the appointment with you.

3. Upload the Physician-completed Section 6 into your Big Team Student Central account.
