Student Information Change Form

Please fill the form out and email it to twagner@jsasd.org. You will need to include proof of residence before your request will be completed. Proof of residence must be one of the following documents, a lease, a deed, current utility bill, vehicle registration, driver’s license or change of address card from PennDOT.
